No clinics have been set up to use this feature.
Please fill in all of the information asked.
It is used to decide how many slots to schedule for your needs and maximize the time we are there.
Once completed you will receive a text within 24-48 hrs to the cell number you provide
with your appointment time.
Please only bring the pets that you have included while filling out this form.
Only those with appointments will be admitted.
Walk-ins or additional animals will not be allowed.
Please be on time and plan to stay with your pet until he/she is taken into surgery. This can be an hour or more. We don't have room to keep dogs prior to their appointment. We appreciate your patience.
In the Health Notes section below please tell us if there are any known issues affecting your pet's health that we need to know about. This is vital info before surgery.
Dogs must be on a leash and cats in a carrier. Please leave pets in your car/truck until we ask for them.
Please don't feed or give water to your pets that are due for surgery past midnight the night before. If they do ingest by accident after that point be sure to tell us at registration.
